Listen to the Bullet Birthday Club each weekday around 7:35am!

Make sure you enter the Name, Age, Birthday and Address of the Birthday person.  Please note that we need the address of the person celebrating their birthday, not your's (the person submitting the entry) if it is different than that of the Birthday person.  This is so we can announce the proper city where that person lives and (should they win) mail them their prize.

Fill out our Bullet Birthday Club Form below.  After clicking the "submit" button... the page will return you to the blank form again... in the event you wish to submit more than one birthday.

After you are finished, just exit that page and all of your entries will go through to us.

Be sure and listen each morning, 7 days a week, for the Birthday Club around 7:35am.
